Serotonin Syndrome: What It Is and When It Is an Emergency

Quick answer

Serotonin syndrome is a reaction to too much serotonin activity, usually when two or more medicines that affect serotonin are taken together. Symptoms can start within minutes to hours and can be deadly if untreated. Contact your provider right away, and never stop a prescription on your own.

What is serotonin syndrome?

Serotonin syndrome is a drug reaction caused by too much serotonin activity in the body. It most often follows a combination of medicines, or a new start or higher dose.

MedlinePlus says it usually occurs when two or more medicines that affect serotonin levels are taken together, such as triptans with SSRIs or SSNRIs (MedlinePlus, Serotonin syndrome). The risk is higher when you first start a medicine or raise the dose.

Most people who take these medicines never have it. The point is to know the pattern so you can act early.

What are the signs of serotonin syndrome?

Look for agitation, diarrhea, a fast heartbeat, hallucinations and a rising body temperature. They can appear within minutes to hours of a new medicine or dose (MedlinePlus, Serotonin syndrome).

Read this as a pattern, not a diagnosis. Only a clinician can confirm it. If several signs appear together after a change, treat it as urgent.

DoctorAny of these is a reason to book a visit. A supplement is not a way to wait, and only a clinician can say what a memory change means.

Signs adapted from NIA (NIA, Forgetfulness: When to Ask) and the Alzheimer's Association (Alz. Assoc., Mild Cognitive Imp.). This is a prompt to talk to a doctor, not a screening test.

Do not wait it out.

Call your provider right away, or emergency services if symptoms are severe. Do not stop your prescribed medicine on your own first.

Can a supplement like St. John's wort cause it?

It can contribute. NCCIH says combining St. John's wort with certain antidepressants or other serotonin-affecting drugs may lead to serious serotonin-related side effects (NCCIH, St. John's Wort safety).

MSK also lists serotonin syndrome as reported with St. John's wort, especially alongside prescription antidepressants (MSK, St. John's Wort). The MedlinePlus page does not itself address supplements, so the supplement link comes from NCCIH and MSK.

St. John's wort is one of six names linked to MemoPryl on a sibling pack. It is unconfirmed for your bottle, and no amount is printed on any image we hold. If you take an antidepressant or a migraine medicine, ask before you start.

What should I check on a supplement label?

Check for a full ingredient list, and read it for any herb that touches mood. If the panel is missing, treat the product as unchecked.

The images we hold of MemoPryl show no Supplement Facts panel and no warnings. Tap the numbered spots to see what the front label shows and what it leaves out.

FAQ01

Quick answers on serotonin syndrome

How fast does serotonin syndrome start?

Usually within minutes to hours of starting a medicine, raising a dose or adding a second serotonin-affecting drug. Symptoms such as agitation, diarrhea, fast heartbeat and fever can build quickly, so call your provider at once if they appear.

Should I stop my antidepressant if I feel strange?

No, not on your own. Call your provider or emergency services right away and describe your symptoms. Stopping a prescription abruptly can cause its own problems, so let a clinician decide what to change.

Is mild shakiness the same as serotonin syndrome?

Not necessarily. Many things cause shakiness, including caffeine and low blood sugar. Several signs together after a medicine change, such as agitation, diarrhea and a racing heart, are the pattern to report quickly.
