Keeping a Supplement Log: A One-Page Method

Quick answer

A supplement log is a one-page record of what you took, when and how you felt, kept every day for at least a month. It lets you and your pharmacist spot side effects, drug clashes and false hopes. Copy the label wording on day one and change one thing at a time.

Why keep a supplement log at all?

Because memory of how you felt is unreliable. A log turns a vague impression into a record you and your doctor can read.

NCCIH advises telling your providers about everything you take, and stopping a supplement and calling your provider if you have side effects (NCCIH, Using Supplements Wisely). A log makes both easy.

It also protects you from guessing. If you change your sleep, your coffee and your capsule in the same week, you cannot tell which one mattered.

What goes on the one page?

Two parts: a header you fill in once, and a daily row. Keep it short enough that you will actually do it.

Header: the bottle name, which label you have, the capsule count, the lot number if printed, the start date and every medicine you take. Copy the directions exactly from the bottle. MemoPryl directions are not printed on any image we hold, so never copy a number from a website.

Daily row for your log
ColumnWhat to write
DateDay and month
Capsules takenCount, as the label says, and the time
SleepHours, and how rested you felt
FocusA score from 1 to 5, or one word
BodyHeadache, stomach upset, dizziness, rash, anything new
Other changesCoffee, new medicine, illness, stress

How do I start the log on day one?

Photograph the bottle and copy the label. Then write a baseline before the first capsule.

Two labels exist, so record which one you have. Label A reads Cognitive Support Formula with 30 capsules. Label B reads Advanced Formula with 60 capsules.

Next step:

Write three baseline lines before the first capsule.

Write three baseline lines: how you sleep, how your focus is on a usual day and what worries you about memory. A month later, the baseline is what you compare against.

Which side effects belong in the log?

Anything new. List it with the date and time, even if it seems small.

Ginkgo is linked with headache, nausea, stomach upset and dizziness, and with a possible rise in bleeding risk (NCCIH, Supplements and Cognition). St. John's wort can clash with many medicines (NCCIH, St. John's Wort safety). Both are named on a sibling pack, though amounts are not printed on any image we hold.

If you see a serious reaction, stop the product and seek medical advice. FDA takes reports through its Safety Reporting Portal (FDA, Report a Supplement Problem). Keep the bottle and your log. Read the side effects page for the warning signs.

How do I use the log with my doctor or pharmacist?

Bring it, with the bottle, to the visit before you start and again after a month.

Keep taking prescribed medicine, and ask your doctor or pharmacist before you start any supplement. Show them the header and the first week. They can spot a clash faster from a list than from memory.

At day 30 and day 60, read your own rows. Did the numbers move? Did the body column fill up? The results timeline gives a calm way to judge, and the 60-day window is the time to decide.

FAQ01

MemoPryl questions people ask

What should I write in a supplement log each day?

Write the date, capsules taken and time, hours slept, a focus score, any new body symptoms and other changes such as coffee or illness. Five minutes at the same time each evening keeps the habit.

Is a phone app better than a paper supplement log?

Use whichever you will keep up. Paper is easy to carry to an appointment, while an app can set reminders. Either way, record the bottle label, the dates and every medicine you take.

How long should I keep a supplement log?

At least a month, and longer if you keep taking the product. A month gives a baseline, a few weeks of data and time to notice side effects. Keep the log for as long as you keep the bottle.
