How to Prepare for a Memory Appointment
To prepare for a memory appointment, write down what has changed and when, list every medicine and supplement, bring the bottles and ask a family member to come. Add your questions. Good notes help a doctor look for causes faster.

What should I bring to a memory appointment?
Bring a medicine list, the supplement bottles, a notes page and a trusted person.
The National Institute on Aging says any concern about memory is a reason to talk with a doctor (NIA, Forgetfulness: When to Ask). The visit is more useful when you arrive with facts, not just worry.
- Every medicine, with doses
- Vitamins, herbs and supplements, in their bottles
- Dated examples of what has changed
- A family member or friend, if possible
- Insurance details and past records
How do I describe what has changed?
Use short, dated examples. Say what happened, when and how often.
| Vague | Useful |
|---|---|
| I forget things | Missed two appointments this month |
| I get confused | Lost on a route I walk weekly |
| My memory is worse | Asking the same question daily since spring |
NIA lists forgetting important events and becoming lost in familiar places as signs to take seriously (NIA, Forgetfulness: When to Ask).
How should I list supplements and medicines?
List everything, including products you think do not count.
FDA says a clinician may need to know about supplements because they can change how prescribed medicines work, and may ask you to stop some before a procedure (FDA, Mixing Meds and Supplements). NCCIH advises telling providers about everything you take (NCCIH, Using Supplements Wisely).
Photograph the label, front and back. Note the date you started each one and why.
Keep taking any prescribed medicine, and ask your doctor or pharmacist before you start a new supplement.

See packsWhat questions should I ask the doctor?
Ask what might be causing it, what tests are next and what you should watch for.
- 01
Read the bottle you receive
Check the safety seal, the capsule count (30 or 60), the directions and the Supplement Facts panel. No image we have shows a panel, so the bottle is the source.
- 02
Check your medicines first
St. John's wort weakens many medicines and ginkgo can raise bleeding risk. Before the first capsule, let your pharmacist see the bottle.
- 03
Take it as the bottle directs
No dose is printed on any image we have. Follow your bottle and never guess a dose from a website, this one included.
- 04
Write a date and a note
Note what you notice and when. If memory or thinking worries you, see a doctor; a supplement is not a way to wait.
Add: Could my medicines or supplements play a part? When should I come back? The Alzheimer's Association says people diagnosed with MCI should be reevaluated every six months (Alz. Assoc., Mild Cognitive Imp.).
What if I take MemoPryl?
Tell the doctor and bring the bottle. It is a supplement, not a memory test or treatment.
MemoPryl is sold for memory and focus, but it is not a test or a treatment for any memory condition. Six ingredient names are linked to a sibling pack, amounts are not printed, and none is confirmed for your bottle. Your bottle, not our images, is the source of truth, so the doctor should read it. The fit check page covers who should ask first.

FAQ01
MemoPryl questions people ask
Who should go with me to a memory appointment?
Bring someone who knows your daily routine, such as a spouse, adult child or close friend. They can describe changes you may miss and help remember what the doctor says.
Do I need to stop my supplements before the visit?
Do not stop anything on your own. Bring the bottles and ask the doctor what to do. Keep taking prescribed medicine unless a professional says otherwise.
How long should I track symptoms before booking?
You do not have to wait. If a change is new, repeated or worrying, book now. Start your dated notes today and add to them before the visit.
