What Is Acetyl L-Carnitine? Definition and Uses

Quick answer

What is acetyl L-carnitine? It is an acetylated form of carnitine, a compound the body makes, sold in capsules for memory and energy. Reviews in dementia are mixed, and doses of about 3 g a day cause stomach upset. Read the label amount first.

What is acetyl L-carnitine?

Acetyl L-carnitine, often shortened to ALC, is a form of carnitine with an added acetyl group.

Carnitine is a compound the body makes and uses to move fats into cells for energy. NIH's fact sheet covers carnitine in general, including supplements (NIH ODS, Carnitine).

The acetyl form is the one sold for brain-related claims. You will see it as "acetyl-L-carnitine", "ALCAR" or "N-acetyl L-carnitine". Same family, slightly different spellings.

A name is not a benefit. Check whether the label gives an amount per serving.

What have studies found about acetyl L-carnitine and memory?

Mixed results, and no firm conclusion. A review says its role in Alzheimer disease and other cognitive disorders is "still under debate" (Pennisi 2020, ALC in dementia).

That review notes a 2003 systematic review found insufficient evidence for clinical use in dementia, while a meta-analysis from the same year reported benefits on clinical scales and psychometric tests. The authors asked for larger, longer, placebo-controlled trials.

NIH adds that a 2003 Cochrane review of 15 trials, using 1 to 3 g a day for 12 to 52 weeks in mild to moderate dementia, found no effect on severity, daily function or global impression (NIH ODS, Carnitine). It concluded routine clinical use was not justified.

What side effects does acetyl L-carnitine have?

At about 3 g a day, carnitine supplements can cause nausea, vomiting, stomach cramps, diarrhea and a fishy body odor (NIH ODS, Carnitine).

NIH also says carnitine can cause seizures in people who have seizure disorders. Valproic acid, phenobarbital, phenytoin and carbamazepine lower blood carnitine levels, which is a reason to talk to a pharmacist if you take any of them.

Seizure history or seizure medicine?

Keep taking any prescribed medicine, and ask your doctor or pharmacist before you start a new supplement.

FAQ01

MemoPryl questions people ask

How does acetyl L-carnitine differ from plain carnitine?

No. Acetyl L-carnitine is carnitine with an added acetyl group. Both relate to carnitine, but studies and labels treat them as different products, so check which name your bottle prints.

Does acetyl L-carnitine give you a fishy smell?

It can. NIH lists a fishy body odor among effects at about 3 g a day of carnitine supplements, alongside nausea, vomiting, cramps and diarrhea. Ask a pharmacist if symptoms bother you.

Can acetyl L-carnitine help with dementia?

Reviews are unsettled. A 2003 Cochrane review found no effect on severity or daily function and judged routine clinical use not justified. See a doctor for any memory worry.
